Please use this checklist to ensure that all details are covered for the March 5-9, 2009, AMSPDC annual meeting:

Go to Registration to register online. You will have the option of paying by credit card or check. This must be completed BY February 12, 2009

On the registration form, be sure to indicate whether you will attend the:

Requests for refunds must be made in writing. If you wish to cancel after paying the registration fee, a full refund will be given when notifying AMSPDC by February 19, 2009. After that time, only a partial fee of $100.00 will be refunded. Refunds will be issued after the completion of the meeting.

Make hotel reservation with Westin BY February 2, 2009. Go to Hotel for reservations and information

Be sure to refer to AMSPDC for the group rate and to insure AMSPDC receives credit for the room.

ANNUAL MEETING ROOM RATE OF $220.00 WILL NOT BE GUARANTEED IF RESERVATIONS ARE MADE AFTER FEBRUARY 2, 2009. THERE IS A LIMITED NUMBER OF ROOMS BLOCKED FOR THIS GROUP; THEREFORE WE RECOMMEND THAT YOU MAKE YOUR RESERVATION EARLY TO ENSURE AVAILABILITY. AFTER ROOM BLOCK IS PICKED UP, RESERVATIONS WILL BE ACCOMMODATED ON A SPACE AND RATE-AVAILABLE BASIS.
